Dental Implants When You Smoke, Have Diabetes or Gum Disease: What an Honest Assessment Looks Like

Ask ten clinics whether you can have dental implants if you smoke, have diabetes or have had gum disease, and you may well get ten confident yeses. A cautious answer should reassure you more. None of these three automatically rules out implants — but each one genuinely changes the risk, the planning and sometimes the timeline, and a clinic that quotes you a price without asking about any of them has not really assessed you at all.

Smoking: higher risk, not an automatic no

An implant succeeds by fusing with your jawbone, and that healing depends on good blood supply. Nicotine narrows blood vessels and reduces blood flow to the gum and bone, which is why smokers, as a group, heal more slowly and have higher implant failure rates than non-smokers. This is standard dental knowledge, not a scare tactic.

It does not mean refusal. Many smokers have implants that integrate well. It does mean the risk should be named openly, and that cutting down — or ideally stopping — for a few weeks either side of surgery gives the implant a better start. Any clinic telling you smoking makes no difference is not being straight with you.

Diabetes: control matters more than the diagnosis

The useful question is not whether you have diabetes, but how well controlled it is. Well-managed diabetes, with stable blood sugar, is generally compatible with implant treatment and normal healing. Poorly controlled blood sugar slows healing and raises the risk of infection, including around a freshly placed implant.

That is why an honest consultation asks about your medication, your recent readings and how stable things have been. Sometimes the right advice is to work with your own doctor on control first and place implants a few months later. A short delay is a far better outcome than a failed implant.

Gum disease: treat the ground before you build

An implant sits in bone and lives surrounded by gum — exactly the tissues that active gum disease infects. Placing implants into a mouth with untreated periodontitis raises the risk of peri-implantitis, an infection around the implant that can destroy supporting bone and cause the implant to fail. Implants cannot decay, but they are not immune to gum problems.

The honest sequence is therefore to treat and stabilise the gum disease first, then place implants. That may add appointments or a second stage to your plan, and it is worth every one of them.
