Bone Grafts and Sinus Lifts: Why Some Jaws Need Preparation Before Implants

Not every jaw is ready for a dental implant on the day the decision is made. An implant is a titanium post that needs a certain volume and density of bone to anchor into, and in some patients — particularly those who lost the tooth years ago — that bone is no longer there. If a dentist has told you that you need a bone graft or a sinus lift before implants, nothing has gone wrong. It is one of the most common findings in implant planning, and it is fixable. Here is what is actually happening, and what it means for your timeline and your budget.

Why bone disappears after a tooth is lost

Jawbone behaves a little like muscle: it stays strong when it is used. The root of a tooth transmits chewing forces into the bone around it, and that stimulation tells the body to maintain the bone. Once the tooth is gone, the signal stops, and the body gradually resorbs bone it no longer thinks it needs. The loss is fastest in the first year after an extraction and continues slowly for years afterwards. Gum disease, long-term denture wear and difficult extractions can all speed the process up.

This is why the length of time a gap has existed matters as much as the gap itself. Two patients missing the same tooth can present with very different amounts of bone.

What a bone graft actually does

A graft rebuilds volume where bone has been lost. The grafting material — which may be synthetic, animal-derived or taken from your own jaw — is placed into the deficient area, where it acts as a scaffold. Over the following months your own bone grows through it and gradually replaces it. A small graft can often be placed at the same appointment as the implant itself. A larger defect usually needs to be grafted first and left to heal for several months before an implant can be placed into it.

When the upper jaw needs a sinus lift

The upper back teeth sit directly beneath the maxillary sinuses, the air spaces behind your cheekbones. Bone here is often naturally thin, and it thins further once molars are lost. A sinus lift gently raises the membrane lining the sinus floor and places grafting material beneath it, creating the height an implant needs. As with grafts elsewhere, a modest lift can sometimes be done at the same time as implant placement, while a larger one needs its own healing period first.

What this means for timeline and cost
